Social Services Coordinator - FT:
Seeking experienced social services professional to coordinate the social service function for assisted living, skilled nursing and memory support. Duties included, but not limited to: social services assessments (and documentation), discharge planning, staff & family education, family and resident support services, active participation on interdisciplinary care team and liaison with outside community agencies. Bachelor’s degree in Social Work or Human Services required; Masters preferred. Must have strong understanding of both state and federal regulations as it relates to long-term care. The successful candidate must possess exceptional organization, time management and interpersonal skills; ability to work with complete autonomy.
Assisted Living Nurse Mgr (LPN/RN) – FT:
Seeking experienced geriatric nurse to direct the day-today operations of the 61-bed Assisted Living and Memory Support units of our Health Pavilion. The successful candidate will be charged with direct patient care, as well as, managerial responsibilities: scheduling, supervising, training, evaluating, teaching, completion of state required documentation for the ALF and Memory Support Units, active participation on interdisciplinary care team and interacts with patients and families. Must have strong understanding of both state and federal regulations as it relates to ALFs. FLA RN/LPN license; two yrs. exp. in a leadership role. ALF CORE training certification required or must complete within 60 days of hire.
Activities Team Leader FT:
Responsible for the organization of an activities program which includes group activities and individual activities for each community member in the Health Center. Accountable for the quality of programs in compliance with legal mandates and under the guidelines of the policies and procedures of Oak Hammock at the University of Florida. Is a qualified therapeutic recreation specialist and eligible for certification as a therapeutic recreation therapist; or has 2 years experience in a social or recreational program within the last 5 years, 1 of which was full time in a community member activities program in a health care setting; or is a qualified occupational therapist or occupational therapy assistant; or is a certified Activity Director through completion of CAD certification course. Excellent communication and interpersonal skills. Valid Florida Driver’s License; must obtain a CDL license to provide transportation to and from off-campus activities as needed. Knowledge of state and federal regulations as they relate to Skilled Nursing and Assisted Living preferred.
Housekeepers - FT & Temporary:
8 am to 4:30pm
Varies; weekends required
Responsible for keeping apartments/homes and its contents
clean (mopping, vacuuming & sweeping, etc), sanitary and orderly, and performs other duties as assigned. Experience required in hospitality or healthcare setting.
