Screening Questions

The following questions will be asked of guests, visitors and private duty staff at the Oak Hammock security gate:

Proof of a recent test is no longer a requirement for entrance. All non-residents must answer the following questions:

    1. Are you experiencing any of the following symptoms that can be related to COVID-19: shortness of breath, cough, fever, sore throat, chills, headache, diarrhea, muscle pain, fatigue, nausea or vomiting, new loss of taste or smell and congestion/runny nose?
      • If yes, entrance is not permitted. 
    1. Within the last 14 days, have you been in close contact with person(s) confirmed or suspected to be infected with COVID-19 who either tested positive or has not been tested?
      • If yes, entrance is not permitted.
    1. Have you tested positive for COVID-19 within the last 10 days?
      • If yes, entrance is not permitted.
    1. Have you traveled outside the United States in the last 14 days?
      • If yes, have you tested negative for COVID-19 since your return?
        • If no, entrance is not permitted
