We take pride in our partnership with Therapy Management Company, which is a clinical service program that combines innovative rehabilitation technologies with customized clinical protocols and pathways, advanced therapist training and ongoing support by licensed physical and occupational therapists and speech-language pathologists.

In addition to typical licenses, our team includes highly educated and credentialed professionals, many of whom hold advanced degrees in nursing care, clinical expertise or are actively pursuing further certifications.

Assisted Living

Designed for those who need help with the activities of daily living, such as dressing, bathing or assistance with medications, Assisted Living at Oak Hammock offers a non-traditional, non-institutional approach to life. 

Residents can enjoy private accommodations, a neighborhood setting, fine dining and customized health care programs. Oak Hammock staff members are dedicated to creating a positive experience through personalized services in a friendly, comfortable and home-like environment of 46 residences.

All residents who live at the Health Pavilion have access to a variety of programs and events, social activities, salon services, transportation services, dining experiences and entertainment. A patient services liaison also is available for specific care needs. 

Skilled Nursing

If you are looking for excellence and innovation in skilled nursing, you’ll find it at the Health Pavilion at Oak Hammock. With low patient-to-staff ratios and reputable star ratings from the state of Florida, Skilled Nursing offers short-term rehabilitation services and long-term care. 

Our skilled nursing community features 73 private accommodations for individuals who are recuperating after surgery or who need rehabilitation following a hospital stay, as well as those who require skilled nursing services over an extended period. We are dedicated to values such as respect, dignity and autonomy. We believe in providing a community where every individual has the opportunity to live a satisfying, purposeful and meaningful life no matter where they are along life’s continuum.

Memory Care

Memory care at Oak Hammock creates a family-style atmosphere that encourages engagement in daily activities and interaction with others. 

Residents with Alzheimer’s disease and other cognitive-related disorders, including various types of dementia, can maintain autonomy and dignity in a safe, secure environment nurtured by specially trained and compassionate staff. Memory care at Oak Hammock is warm, welcoming and comforting for residents, families and loved ones.

Visitation Policy

Oak Hammock at the University of Florida, Inc. is committed to upholding the rights, dignity, and well-being of all residents by ensuring safe, equitable, and consistent access to visitation. The organization recognizes that visitation is a critical component of resident-centered care and quality of life. We affirm that residents have the right to receive visitors of their choosing at any time, subject only to limited and clearly defined clinical and safety exceptions. Visitation shall not be restricted based on vaccination status, disability, race, color, national origin, religion, sex, gender identity, sexual orientation, or payment source. Oak Hammock at the University of Florida, Inc., is committed to following federal and state guidelines, including nationally recognized sources for infection prevention and control.

 

Individuals who wish to file a complaint regarding health care services or facilities may do so by contacting the Florida Agency for Health Care Administration, or AHCA. Complaints can be submitted through AHCA’s official website, where detailed instructions and appropriate forms are provided for various types of concerns, including those related to licensed facilities and regulatory compliance.
